Bonita D. Formwalt

Bonita D. Formwalt, a resident of Glen Burnie for 32 years, passed away suddenly on December 3, 2014. Born in Baltimore, MD to Phillip Dvorak of Connecticut and Dorothy (Wallace) Shanklin of Columbia. Bonita was a supervisor for Worker's Compensation and enjoyed reading and writing in the Baltimore Sun as well as the Maryland Gazette. She loved her family and centered her life on them. In addition to her parents, she is survived by her stepfather, Ervin Shanklin of Columbia; sons, Travis W. Formwalt of Glen Burnie, Gregory R. Formwalt of Glen Burnie; sisters, Patricia Dvorak of Columbia, Danielle Upright of Columbia; half-brothers, Phillip Dvorak of Florida, Scott Dvorak of Florida; half-sister, Jesalyn Dvorak of Washington and grandchild, Olivia A. Formwalt.
